Just ask Mr. Hyde about the first time Cap ran into him after Hyde tortured Jarvis--Cap was so pissed that he was smashing the edge of his shield into all sorts of vulnerable areas. I went back and looked, and Cap hit him in the throat, the gut, the bridge of the nose, the 'nadular region, and the back of the neck. All with an adamantium shield, because he was it his black-suited Captain identity at the time. (Issue #340) Pissing Cap off just isn't a good idea, even when you're a monster like Hyde.
Well, Cap grew up during the Depression, and back in those days people couldn't afford the sort of large extravagant feet that you kids take for granted today.
They took the tiny dainty feet they got and were grateful.
I think it is important to note Cap is "ankling" Hulk in the 'nads, not using the more egregious steel boot tip. The motion is the same, but an ankling is less likely to cause long-term damage.
Cap was right to do it too: you can't leave the house in skin-tight jorts and expect to stay unkicked.
